• Account Numbers: Include any and all Customer ID Numbers so we can link all of your accounts. If you purchase through more than one Authorized Distributor, include the other account number in the “Relevant Details” section.

• Quarter 1 (January – March): End Date – March 31 | Rebate Check Sent Mid July

• Quarter 2 (April – June): End Date – June 30  Rebate Check Sent Mid October

• Quarter 3 (July – September): End Date – September 30 | Rebate Check Sent Mid January

• Quarter 4 (October- December): End Date – December 31 | Rebate Check Sent Mid April
